To submit your application electronically, fill in all blanks below, and click the submit button. You will be notified by mail that your application has arrived. You may also email a short message to us, requesting that we send you an application (be sure to include your full name and address!), use our Request Form to have an application mailed to you, or, you may print a paper copy of the form, to be submitted by mail or fax. The Scholarships and Award Committee reviews qualified applicants in September of each year and selects award recipients by letter ballot. Applicants are selected based on five broad criteria. These include:
